New Patients Registration, Temporary Residents & Catchment Area  


We accept patients living within approximately a 6 mile radius of Aylsham and Reepham. 


 Registering Patients



When you join the practice you will be required to complete a registration form and a new patient questionnaire.  You will need to provide us with two forms of identification.  When you register with the practice you will be allocated to one of our doctors who will be responsible for your care. 


Please note: If you are registering at our Reepham practice your registered doctor will be a Reepham doctor, and if you are registering at our Aylsham practice your registered doctor will be an Aylsham doctor.


All new patients who are adults should book in for a new patient check with our Healthcare Assistant. This will enable us to check blood pressures, offer health advice and take details of any repeat medications that you are taking. Please bring a urine specimen with you to your appointment you will be given a container when you register at the practice.  You may need to see a clinician if you need any prescriptions.

If you use the printed template please ensure you speak to a member of the reception team in the surgery to fully complete your registration process; we require you to complete a GMS1 form before we can register you which can only be retrieved from the practice.



Temporary Residents


If you need to register temporarily with our practice you can request a temporary resident form from reception, the form requires a permanent home address and a temporary address along with personal demographics. You can register temporarily to receive immediate necessary treatment, up to 16 days or 16 days - 3 months. By registering as a temporary resident your care will not end at your registered GP surgery, if you are seen by our practice your records will be updated accordingly.



Named GP for all patients


You may be aware from the 1st April 2015 all practices are required to allocate all their patients with a named GP who will have overall responsibility for your care and support.


Having a named GP does not prevent you from seeing any other GP in the practice.


Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor. All of your records are available to every GP in the surgery.


We have made every effort to make patients aware of who their named GP is. If however you are unsure please ask the receptionist team at the practice.


This notice applies to all of our patients including children. Patients over 75 have already been informed of their named GP.










Leaving The Practice



If you are a patient registered at Reepham & Aylsham Medical Practice and move out of our catchment area you will be required to register at a practice closer to your new address. 


You will have 28 days after you move to register at another practice within your new area. 


Once you have left our practice and registered elsewhere we will no longer have access to your medical records as these will be transfered directly to your new practice.
