If you are asked to bring in a specimen, you need to ensure you have collected the correct container from reception. The specimen needs to be returned on the day it is collected, by 2.30pm at Reepham and 3pm at Aylsham, as it may need to go off to the laboratory for further testing. Please ensure the specimen container is clearly labelled with the patient details. If this is not completed or clear the specimen will not be tested. Please only bring in a specimen at the request of a clinician or if this is part of your annual review.
Please remember the Practice is closed 1-2pm and on the last Tuesday afternoon of every month.
Screening services for AAA is available to all males (and individuals assigned male at birth) over 65yrs of age. Invitations to attend will be sent by post – please make sure your contact details are kept up to date with us to ensure this (and other) health information and invites are received. If you are planning to travel outside the UK, you may need to be vaccinated against some of the serious diseases found in other parts of the world. Prior to us providing any travel vaccinations, we will need you to complete an online travel risk assessment questionnaire.
If you are unable to complete our online form, you can request a paper copy by visiting either surgery during our opening hours.
Once your assessment form has been reviewed, we will contact you directly to book an appointment should further action be required.
Please note, not all vaccinations are available for free on the NHS even if recommended for certain areas. We will advise you if this is the case and provide information on alternative providers. This is a simple test used to help diagnose and monitor certain lung conditions. If this is required, we can perform this test at either of our surgeries and will inform you directly with more details on how to book.

We have regular appointments with first contact physiotherapists (FCP) who can assist with an initial review relating to most musculoskeletal (MSK) issues such as back, neck and/or joint pain. FCPs can complete a full assessment, diagnose, give expert management advice, and refer to other services if required.
FCPs are not able to see patients for on-going support. They can refer patients to a full physiotherapy service if required.

Our GPs are trained and specialise in minor surgical procedures sometimes under a local anaesthetic such as removal of moles, warts and cysts, joint injections, and treatment of ingrowing toe/fingernails. Please book a routine appointment to discuss any concerns further.
There are different staff available within the practice who can support you with your mental health and wellbeing. Who you see will depend on individual circumstances.

If you are off sick for less than seven days, your employer should not ask for medical evidence that you have been ill. Instead, they can ask you to confirm that you have been ill. You can do this by filling in a form yourself from them and when you return to work (also called self-certification).
If you are off work sick for more than seven days your employer will usually ask for a medical certificate (sick note). To request a sick note:

Please note: A medical certificate can be backdated if required, please do not request an emergency or urgent appointment for this reason.
If you have been discharged from hospital, your hospital team will issue any sick notes for the entire period of your recovery. Please contact the ward staff (even after discharge). Schools should not request a GP letter to confirm absence, this can be written by a parent or guardian.

Our GPs and nursing team want to support you to live well with any long-term condition. The monitoring required will depend on your individual circumstances. Usually, this takes place annually although some conditions or medications may require more frequent monitoring.
Some reviews may not require you to be seen and can take place via other communication routes.
We will contact you directly, usually around your birth month to arrange and advise you of the required actions we need you to take e.g., blood tests, urine samples, diaries, questionnaires.
